Curt Lehman Memorial Scholarhip Form
Upon acceptance to the CLC, students are able to apply for a scholarship worth up to one-half of full-time tuition.
Application for Graduation
Completed applications must be returned to the CLC office in order to be considered for spring graduation. Deadlines occur in late February of each year.
Student Request for a Grade of Incomplete
Student must complete this form and submit it to the instructor of the class for which the Incomplete is being requested. Student must have a passing grade in work already completed and have completed a substantial portion of the work in the course.
Request for Official Transcripts
A student may request their CLC transcript to be sent to another institution of higher learning by completing this form. No requests will be granted by phone or e-mail correspondence.
Request for Withdrawal or Leave of Absence
The maximum length of time for a leave of absence is one calendar year. Students who do not return within one year are withdrawn and will need to reapply by submitting a Return Student Application.
Returning Student Application
A Returning Student Application must be completed by a CLC student who has taken a leave of absence that extended beyond the one-calendar year limit. The completed Returning Student Application must be submitted to the CLC office at least one-month prior to date of re-enrollment.
