Scholarship Form
You may apply for a scholarship to the CLC by downloading the form and submitting it to the CLC office.Notification of scholarship award will be made in writing. Please allow up to four weeks for notification.

Request for a Grade of Incomplete
Initiate a request by filling out the Request for a Grade of Incomplete form and submitting it to the course instructor for which the Incomplete is being requested. You must have a passing grade in work already completed and have a substantial portion of the work in the course completed before a request can be made.
Application for Graduation
Completed Application for Graduation form must be received in the CLC office by February 27, 2009 for spring graduation. Failure to submit an Application for Graduation will result in the student not being included in the ceremony.Request for Withdrawal or Leave of Absence
Students requiring or desiring a leave of absence or withdrawal from the CLC must submit a Request for Withdrawal or Leave of Absence form to the CLC.
The maximum length of time for a leave of absence is one calendar year. Students who do not return within one year are withdrawn and will need to reapply.
Request for Official Transcripts from the CLC
You may request your official transcripts from the CLC to be sent to another institution upon submission of the Request for Official Transcripts form to the CLC office.
Transcripts will be released to the student or another institution with written permission from the student. Phone or email requests will not be granted without the proper document and signature on file in the CLC office.
